Seven Possible Causes of Hand Tremors: When Shaking May Signal a Health Problem

Hand tremor is a relatively common symptom. It can occur temporarily because of stress, fatigue or excessive caffeine consumption, but persistent or worsening tremor can also be associated with neurological, metabolic or other medical conditions.

Here are several possible causes.

1. Parkinson’s Disease

A tremor associated with Parkinson’s disease commonly occurs when the affected hand is at rest and may initially be more noticeable on one side of the body.

A characteristic movement sometimes resembles rolling a small object between the thumb and fingers. However, tremor alone is not enough to diagnose Parkinson’s disease.

2. Essential Tremor

Essential tremor is a common movement disorder that can run in families.

Unlike the classic resting tremor associated with Parkinson’s disease, essential tremor is generally more noticeable when a person is using their hands—for example, while writing, eating or holding a cup.

3. Overactive Thyroid

Excess thyroid hormone, known as hyperthyroidism, can cause a fine hand tremor.

Other possible symptoms include a rapid or irregular heartbeat, increased sweating, anxiety, heat intolerance and unexplained weight loss.

4. Low Blood Sugar

A significant drop in blood glucose, or hypoglycemia, can cause shaking, sweating, weakness, hunger, palpitations, dizziness and confusion.

This can be particularly important for people with diabetes who use insulin or certain glucose-lowering medications. Severe hypoglycemia requires prompt treatment.

5. Severe Liver or Kidney Disease

Advanced liver or kidney dysfunction can affect the nervous system when substances normally processed or eliminated by these organs accumulate in the body.

Some patients may develop abnormal hand movements, although these are not always a typical rhythmic tremor and should be medically evaluated.

6. Conditions Affecting the Cerebellum

The cerebellum plays an important role in coordinating movement. Stroke, multiple sclerosis and other conditions affecting this part of the brain can cause an intention tremor, which may become more pronounced as the hand approaches a target.

7. Stress, Anxiety and Excessive Caffeine

Stress, intense anxiety, panic and excessive consumption of coffee, energy drinks or other stimulants can temporarily make normal physiological tremor much more noticeable.

Reducing caffeine, getting adequate sleep and managing stress may help when these are the primary triggers.

Hand tremor has many possible causes, including certain medications, and its pattern, timing and accompanying symptoms are important for determining the cause.

If tremor appears suddenly—especially together with weakness, facial drooping, speech difficulty, severe dizziness or other neurological symptoms—urgent medical attention is necessary. Tremor that gradually worsens or interferes with everyday activities should also be evaluated by a healthcare professional.
